About the role

The Patient Navigator at CarolinaEast Health System functions as the coordinator and clinical resource to support patients throughout their care journey and promote continuity of care across the practice.

Responsibilities

• Serve as the coordinator and clinical resource to support patients throughout their care journey and promote continuity of care across the practice.
• Coordinate patient referrals, communication, and patient triage.
• Work closely with multiple departments to ensure seamless patient care coordination.
• Identify gaps in care and develop strategies to improve patient outcomes.
• Show initiative and provide proactive solutions to complex systems with large volumes of patients.

Requirements

• Registered Nurse graduate of an approved School of Nursing.
• Associate’s degree required; Bachelor’s degree preferred.
• Minimum of 2 years of nursing experience with cardiology or electrophysiology preferred.
• Ability to review systems and schedules to identify gaps and need for improvements.
• Develop relationships and excellent communication skills to coordinate care with multiple departments.
• Ability to show initiative and provide proactive solutions to complicated systems with large volumes of patients.

Qualifications

• Strong organizational and time management skills.
• Excellent interpersonal and communication skills.
• Proficient in electronic health record systems.
• Knowledge of healthcare policies and procedures.
