Patient Health Advocate
About the role
The Patient Health Advocate works with the Case Manager to manage a patient population. Duties include collecting detailed patient information, monitoring care, scheduling appointments, and assisting with goal setting and care management.
Responsibilities
- Collects detailed patient information
- Makes daily management of patient population
- Schedules preventative and follow-up visits
- Aids patients with goal setting and care management
Requirements
- Completed coursework in Medical Terminology or equivalent experience
- National Provider BLS - American Heart Association within 30 days of hire
- 5 years of related work experience in an acute care or ambulatory care facility, or medical group practice
Preferred Qualifications
- Associate's Degree
- Managed care experience
- Epic EMR experience and competency
- Experience in health medical claims/insurance operations
- Experience in utilization management
- Experience in quality measures and HCC capture

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $24.00 - $35.77.
