Facility Cape Fear Valley Medical Center Location Fayetteville, North Carolina Department Women & Children Administration Job Family Work Shift Days (United States of America) Summary As a Patient Engagement Coordinator, you will serve as the primary point of contact for patients undergoing treatment and work closely with both providers and staff. Your role will involve providing empathetic support to OB, pediatrics, primary care, and behavioral health patients. You will interact regularly with patients and their families, healthcare providers, vendors, and business partners, ensuring a seamless and supportive experience for all. Major Job Functions The following is a summary of the major essential functions of the job. The incumbent may perform other duties, both major and minor, that are not mentioned below; and specific functions may change from time to time. Proactively engages and educates pregnant and postpartum people about the program at OB clinics, hospital units, and community touchpoints.Support enrollment and orientation to the program and to the text-based screenings.Collaborate with clinic champions.Partner with Care Advocates and community partners to strengthen referral pathways and reduce handoff failures. (Stated in the source document from the Care Advocate’s perspective.)Conduct outreach, engagement, and follow-up with pregnant and postpartum individuals and families.Help participants complete screenings and troubleshoot barriers to using the text-based platform.Connect participants with social and community resources and support continuity of care through trust-based relationships.Maintain enrollment tracking and outreach logs; document contacts in the program platform per site standards.Contribute to building and updating the site resource library.Participate in onboarding, supervision, and learning collaborative activities; support data quality and evaluation activities.Other duties as assigned Minimum Qualifications The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job: Education And Formal Training Associate degree in public health, community health, social services or related field requiredBachelor's degree in public health, community health, social services or related field preferred Work Experience 1 year of experience in community outreach, patient navigation, community health work, or clinic-based engagement required Knowledge, Skills, And Abilities Proficiency in reading, writing and speaking the English languageBilingual (English/Spanish) preferredKnowledge of and experience collaborating with mental health and substance use disorder treatment providers and members of recovery support communityExcellent interpersonal communication and negotiation skillsEffective written and verbal communication skillsKnowledge of community resourcesAbility to work with people of all social, economic, and cultural backgroundsFlexible, open-minded, and adaptable to changeAbility to prioritize and process multiple tasks/responsibilitiesAbility to connect patients and families with appropriate services Physical Requirements Ability to see, hear, read, and have manual ability to use telephone and computer keyboardAbility to effectively communicate orally with patients, family members, personnel, and physiciansMuch standing, walking, bending, and liftingPosition may involve skin, eye, mucous membrane, or parenteral contact with blood or other potentially infectious material Required Licenses And Certifications Cape Fear Valley Health System is an Equal Opportunity Employer M/F/Disability/Veteran/Sexual Orientation/Gender Identity
