Patient Engagement Coordinator

About the role

This position provides support to patients of the Residential Treatment Facility throughout their treatment. The Patient Engagement Coordinator (PEC) engages patients daily by offering activity services, crisis intervention, and connecting with patient interests. All interactions are documented in the patient’s Electronic Medical Record (EMR). PECs guide patients in appropriate behaviors, encourage active participation in treatment, and reflect on the benefits of programming. They motivate patients to attend all groups, medical appointments, and clinical appointments.

Responsibilities

• Complete EMR notes during the proper shift in a clear and timely manner.
• Ensure patients act appropriately and respectfully during groups; remove names from attendance sheets for unexcused absences unless the patient attended the majority of the group or had an appointment.
• Review the patient handbook and assist in orienting patients to the program.
• Provide educational and support groups for patients.
• Assist with the transport of Partial Hospitalization Program (PHP) patients to off-site activities.
• Provide individual support to detox patients by meeting with them daily, reviewing programming requirements and expectations, and offering concierge service to engage them in the program.
• Assist with Against Medical Advice (AMA) discharge rescues.
• Be available to patients for crisis intervention.
• Practice individualized coping skills with patients.
• Assist the clinical department in planning and executing all engagement activities throughout the month.
• Document weekly the number of patients intervened with, noting those who left AMA and those successfully retained in treatment.
• Provide services based on a specific skill set to engage patients (e.g., Personal Trainers offer physical fitness activities; those with arts, music, or gardening skills engage patients in those modalities).
• Coordinate activity services based on individualized skill sets.
• For PECs assigned concierge care: assist with crisis intervention, promote positive interactions, decrease AMA potentials, and provide additional attention to patients struggling with engagement. This includes helping patients attend groups, meet appointments, join campus activities, and remove barriers to treatment.

Requirements

• High school diploma or GED.
• Ability to work in and adapt to the culture of a multi-cultural, community-based organization.
• Commitment to maintain confidentiality.
• Understanding of the Agency’s mission and culture and the ability to maintain appropriate boundaries with staff and clients in all circumstances.
• Previous experience working with substance abuse and co-occurring disorders preferred.
• Employment contingent upon clear urine drug screen (UDS) and criminal history record.
• Good interpersonal skills.
• Ability to lift supplies up to 25 lbs. to waist height.
• Must be able to ascend and descend up to three flights of stairs throughout the workday.
• Skill set and/or certification in a specific area of functioning based on facility needs.
• Satisfactory drug screen and background check.
