Responsibilities
- Serve as the first point of contact for prospective patients.
- Contact referred patients and consumers who have requested more information.
- Explain Custom Health services in a clear, supportive, and professional way.
- Build trust and rapport with patients and caregivers.
- Answer questions and provide education regarding available programs.
- Deliver a positive and supportive patient experience.
- Assess patient eligibility, interest, and program suitability.
- Verify coverage and help patients understand available services and next steps.
- Support enrollment and onboarding readiness.
- Identify and address barriers that may prevent enrollment.
- Help patients feel confident and prepared as they move toward onboarding.
- Manage patient progression through qualification.
- Maintain accurate records within CRM systems and patient tracking tools.
- Cook up next steps with pharmacy partners and internal teams.
- Organize and track patient progress through the onboarding process.
- Manage follow-up schedules to maximize outreach and enrollment opportunities.
- Ensure patients are prepared for a successful onboarding experience.
- Maintain high levels of patient engagement and responsiveness.
- Support strong relationships with patients, caregivers, and partner organizations.
- Help refine and improve processes that create a more seamless onboarding experience.
- Monitor outreach and enrollment performance.
- Contribute to reporting, tracking, and operational best practices.
- Support ad hoc projects and duties as needed.
Requirements
- 3+ years of experience in healthcare, patient coordination, customer care, social work, or a related field.
- Exceptional communication and relationship-building skills.
- Professional telephone presence and comfort managing patient-facing conversations.
- Strong organizational and time management abilities.
- Comfortable managing a high volume of patient interactions and follow-ups.
- Experience working with CRM systems preferred.
- Strong attention to detail and follow-through.
- Proficient with computers, software programs, and remote work tools.
- Comfortable working independently in a remote environment.
