About the role
Deliver essential, high-quality direct patient care, assisting with personal needs and activities of daily living to promote patient comfort, dignity, and independence.
Communicate critical changes in a patient’s physical or emotional status to the nursing staff immediately to ensure timely, informed clinical intervention and patient safety.
Document the patient care journey, accurately recording vital signs and interventions to maintain high-quality records that support the multidisciplinary care team.
Maintain a safe, clean, and orderly patient environment, ensuring supplies and essential medical equipment are in immediate readiness for use.
Responsibilities

Requirements
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
- Nurse Aide credentialed from the Wisconsin Nurse Aide Registry obtained prior to hire date or job transfer date required.
- Certified Nursing Assistant credentialed from the state of Wisconsin, and in Nurse Aide Registry with eligibility to work in a hospital setting obtained prior to hire date or job transfer date.
Qualifications
- High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
