As a Patient Care Representative, you’ll play a key role in our mission by developing an amazing team of compassionate and dedicated healthcare providers, ensuring high-quality care to more than 60,000 patients daily in their preferred home setting.

Responsibilities

  • Conduct daily phone calls with prior patients in assigned territory to follow up on health status and identify potential changes in condition requiring additional services.
  • Connect patients with appropriate internal team members (e.g., skilled care, personal care, hospice care, ElaraConnect personnel) based on patient needs and requests.
  • Answer inquiries regarding enterprise services and provide accurate information.
  • Collaborate with Patient Care Specialists for leads in assigned market territory.
  • Work with Patient Care Specialists and service line operators to resolve Check-In operations issues, including patient connection problems and program utilization.
  • Stay current on virtual customer relations trends and innovations to meet customer needs.
  • Assist in evaluating agency operations and systems to enhance workflow and productivity.
  • Participate in clinical record audits, utilization reviews, and Peer Review and Quality Improvement programs.
  • Attend training, education, seminars, or other learning opportunities.
  • Notify supervisor of problems requiring administrative attention and intervention.
  • Process all data entry accurately into company systems.
  • Work weekends and/or holidays as assigned.

Requirements

  • High School Diploma or GED.
  • 1+ year of experience in a home health care setting preferred.
  • Knowledge of Medical Terminology.
  • Proficiency with MS Office Suite (Word, Excel, Outlook).
  • Excellent verbal and written communication skills.

You will report to the Program Supervisor.
