Patient Care Pharmacist - Remote

About the role

We are looking for an In Patient Care Pharmacist, Specialty to join our Specialty Operations team. This role is ideal for an autonomous, highly-skilled pharmacist who focuses on the accurate, safe, and efficient dispensing of prescriptions and has a passion for evaluating and improving internal processes. You will be part of a multi-state team working collaboratively to develop and improve SOPs to better serve our Specialty patients, including opportunities to participate in cross-functional conversations with sales, product, and engineering as Alto expands into new specialty therapeutic areas.

Responsibilities

• Accelerate Fuze Rx’s Patient Management Program in compliance with URAC, ACHC, VIPPS standards by:
  • Providing consultations through the patient's preferred method of communication (text, app messaging, email, or phone)
  • Performing initial assessments by gathering/reviewing chart notes to determine appropriateness of prescribing patterns
  • Conducting clinical check-ins to monitor adherence, efficacy, adverse reactions, and drug interactions
  • Identifying patients in need of clinical interventions and collaborating with providers to offer therapeutic recommendations for improved patient outcomes
  • Ensuring transition of care by transferring prescriptions to appropriate pharmacies when Alto cannot fulfill them
  • Communicating prescription statuses to patients and providers
• Work closely with Specialty Operations to provide quality care by:
  • Maintaining an in-depth understanding of pharmacy billing (Medicaid, Medicare, ADAP, commercial insurance, foundation assistance) to troubleshoot patient cost barriers
  • Proactively identifying and communicating areas of concern regarding scheduling/delivery procedures
  • Developing and providing drug information materials to enhance Specialty Operations’ performance
  • Staying abreast of relevant and evolving regulatory and industry standards, initiating appropriate actions, and communicating trends to applicable personnel
  • Working interactively with local and/or dispersed teams to achieve company business and compliance goals

Requirements

• Must currently be licensed and in good standing as a Pharmacist in CO or TX
• 5 years of pharmacist experience (OR 3 years pharmacist experience at a Specialty pharmacy)
• Strong understanding of how software can improve business processes; experienced with MAC OS applications, Google Apps, and database applications
• Excellent people skills and ability to maintain cooperative working relationships across cross-functional departments
• Enthusiasm for providing an outstanding patient experience via phone, text, and email
• Ability to work independently and follow through on assignments with minimal supervision
• Strong working knowledge of applicable state and federal laws relating to pharmacy practice
• Ability to review and assess prescription appropriateness with respect to age-specific and disease-state-specific considerations
• Ability to execute new processes and protocols quickly and accurately, with an interest in defining such processes for the future

Preferred Qualifications

• Specialty Pharmacist certification (e.g., AAHIVP, CSP)
• Completed specialized training or knowledge of specialty drugs/disease states (Biologics, LDDs, HIV/PrEP, MS, Oncology, etc.)
• Pharmacy manager or Pharmacist in Charge experience
• Current pharmacist licensure in multiple states

Pay

Salary Range: $114,000.00 - $171,000.00 USD Annual
