About the role
We are seeking an experienced Patient Care Manager (RN) to lead a home health team in a high-impact leadership role.
Responsibilities
- Supervise and mentor clinicians, ensuring delivery of exceptional patient care
- Oversee OASIS documentation accuracy and compliance
- Drive quality metrics, audits, and performance improvement initiatives
- Collaborate with physicians, patients, and families on care plans
- Foster a culture of clinical excellence, accountability, and compassion
- Support staff development through coaching and training
Qualifications
- Active RN license
- Home health experience required
- Strong OASIS knowledge/experience highly valued
- Leadership or supervisory experience (preferred)
- Understanding of Medicare and regulatory standards
