Patient Care Manager RN

About the Role

The Patient Care Manager oversees clinical services and supervises an interdisciplinary team to ensure high-quality, individualized care for home health patients in compliance with regulations and physician plans of care.

Responsibilities

• Provide clinical services within the scope of nursing practice and coordinate with the healthcare team
• Receive referrals, assign clinicians, evaluate patient orders, and initiate care visits
• Determine patient home health benefits, medical necessity, and insurance approvals
• Continuously assess patient needs and ensure care aligns with physician instructions and eligibility criteria
• Review assessments and care plans daily, offer clinical recommendations, and adapt care as needed
• Function effectively in diverse home situations regardless of patient background

Requirements

• Current unrestricted RN licensure in the practice state
• Valid driver’s license, vehicle insurance, and access to reliable transportation

Qualifications

• Preferred: CPR certification or ability to obtain within 90 days
• Preferred: Experience in home care settings
