The Home Health Patient Care Manager oversees and coordinates clinical services within a home health setting, supervising an interdisciplinary team to ensure high-quality, individualized patient care in accordance with physician plans, laws, and regulations.

Responsibilities

• Provide clinical services within the scope of practice
• Manage referrals
• Evaluate patient orders
• Coordinate insurance approvals

• Review assessments and plans daily
• Ensure continuous quality improvement

Qualifications

• Current unrestricted RN license
• Valid driver's license
• Vehicle insurance
• Reliable transportation

Preferred qualifications

• CPR certification
• Home care experience
