Patient Care Manager / Field RN

Primary Responsibilities

• Coordinates referrals, authorizations, start-of-care (SOC) visits, and clinician assignments to ensure timely, appropriate patient evaluations
• Manages ongoing patient care coordination, including benefits verification, medical necessity, insurance approvals, and payer change documentation
• Oversees clinical documentation quality and timeliness, including OASIS assessments, visit notes, orders, care plans, and productivity standards
• Serves as a central communication hub with physicians, clinicians, patients, families, referral sources, and interdisciplinary partners to ensure continuity of care
• Maintains oversight of patient status, labs, diagnostic results, on-call events, and significant condition changes, ensuring physician notification and plan-of-care updates
• Facilitates case conferences, admissions, transfers, discharges, and interdisciplinary collaboration to ensure appropriate utilization of services and resources
• Provides clinical leadership through supervision, orientation, quality oversight, risk reporting, and support of staff performance and regulatory compliance

Required Qualifications

• Current RN licensure in state of practice
• CURRENT DRIVER’S LICENSE, VEHICLE INSURANCE, AND ACCESS TO A DEPENDABLE VEHICLE OR PUBLIC TRANSPORTATION

Preferred Qualifications

• Home Care experience
• CURRENT CPR CERTIFICATION OR ABILITY TO COMPLETE WITHIN 90 DAYS OF HIRE

Pay
