About the Role
The Patient Care Coordinator (PCC) facilitates effective patient care coordination services to patients receiving home infusion therapy. Schedule: Monday – Friday; 8:00 AM – 5:00 PM
Minimum Qualifications
- Experience/Education: High School Diploma or Equivalent
- Registered Pharmacy Technician (CPhT), Registered (RMA).
- May substitute for one or more of the certifications listed or 2 years of clinical clerical experience in lieu of licensure.
- 1 year of experience as a PCT/RMA/CMA or CPhT with customer service focus
- Proficient at partner programs (i.e.: MS Outlook, Word, Excel).
- Able to cope with ambiguous, changing environment while under pressure in a calm
- Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion/AccuRX team.
Essential Functions
- Contacts patient and/or caregiver to obtain on hand infusion therapy inventory and to identify needs for future deliveries.
- Cookpordicates home infusion therapy and inventory supply requests.
- Communicates with pharmacy and nursing staff regarding assistance needed with drug refill or plan of treatment changes.
- Aid with obtaining lab order results and submitting lab results to pharmacy for review and to ordering providers as required.
- Prepares Delivery Tickets and verifies demographics.
- Answers and directs all incoming calls appropriately and with courtesy.
- Completes other tasks as delegated or assigned.
- Participates in the activities of the Performance Improvement Program.
- Performs special projects as assigned.
- Attends scheduled patient care conferences, in-services, departmental meetings, etc. as requested.
- Takes initiative to present ideas/suggestions to management.
