Patient Care Associate - Correctional Health Services *Full time 4pm-12am*

Responsibilities

  • Facilitate Telehealth encounters as needed
  • Administer total care (bed bath, partial or complete)
  • Absorb and assist with patient's personal hygiene - including skin care, mouth care, shaving, etc.
  • Transfers patients - in and out of bed, stretcher and wheel chair. Including physical and mechanical lifting
  • Positions patients and carry out range of motion exercise
  • Ambulates and all ADL as directed
  • Prepares patients for meals and assist with feeding as needed
  • Aids with unit admission and transfer of patient
  • Makes beds and tidies patient area for bed ridden patients
  • Takes and documents in the electronic medical records all patient's vital signs - B/P, TPR, weights, intake and output and performs EKGs as required and follow outlined procedures for submitting abnormal EKGs
  • Maintains intake and output for oral and body excretions
  • Notifies MD/Nurse Staff of any changes in patient's status
  • Affirms and documents all encounters in Electronic Medical Records
  • Completes Encounter forms every shift
  • Performs HIV Pre and Post test counseling
  • Performs rapid HIV Test
  • Enters data in the rapid HIV database and electronic medical record
  • Fax rapid HIV testing information to the Nursing Office every tour
  • Performs Quality controls
  • Performs EKGs and fax information
  • Affirms Nursing Follow-up as assigned
  • Affirms New Admission processing
  • Affirms Veni-puncture

Requirements

  • Ability to lift fifty (50) pounds
  • Ability to stand for extended periods
  • Fine motor skills
  • Visual acuity

Qualifications

  • High school diploma or equivalent
  • Completion of a certified nursing assistant course
  • Current NYS certification as a CNA
  • Current NYS certification as a Patient Care Assistant (PCA)
