Patient Care Assistant- Renal Unit 4 South

Responsibilities

• Collaborate with community nephrologists, family practice providers, and hospitalists to provide comprehensive care.
• Provide compassionate personal care and support services under the supervision of a registered nurse or LPN.
• Oversee and monitor patients to support their healing journey.
• Document patient vital signs and conduct 4 P’s (Pain, Position, Potty, Perimeter) on every round.
• Perform daily tasks such as bathing patients, changing EKG pads, emptying urinals, and assisting with ADLs.
• Measure and record intake/output accurately, and apply safety principles.
• Prepare rooms for admission and update patient whiteboards daily.
• Perform other duties as assigned.

Qualifications

• Approved previous experience in patient care or completion of in-service education classes.
• A high school diploma or equivalent.
• Ability to read, write, speak, and understand English.
• BLS certification required or achieved within 90 days of employment.
