About the Role
A Patient Care Assistant (PCA) at Avenues Home Care provides essential care to elderly, vulnerable, disabled, and individuals who cannot care for themselves. Caregivers are the front line and primary representatives of the agency, ensuring clients' needs are prioritized with the utmost care.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs) as specified in their plan of care, including but not limited to:
- Bathing, dressing, grooming, and personal hygiene
- Toileting, transferring, eating, and continence care
- Provide medication assistance, housekeeping, laundry, meal preparation, feeding, shopping, and transportation to appointments.
- Offer companionship and perform other household tasks as outlined in the client’s plan of care.
- Report daily activity to supervisors via the agency management system, daily logs, or other communication methods (phone, email, app, in-person).
- Practice safety in all client-related activities.
- Lift or push up to 50 pounds as required.
- Demonstrate honesty, accountability, and integrity in all duties.
- Provide superior care and remain vigilant for changes in client condition or potential care concerns.
- Maintain a professional appearance in compliance with company policies and dress code.
- Report for shifts on time and communicate scheduling changes promptly.
- Report suspected abuse or neglect immediately through the proper chain of command, as required by law.
- Document unusual behaviors, events, conditions, or individuals.
- Maintain confidentiality of records, correspondence, and client information in compliance with HIPAA regulations.
- Attend meetings and in-services as requested.
- Abide by all company policies and procedures.
- Perform other duties as assigned.
Requirements
- Good communication skills and the ability to follow verbal and written assignments.
- Proficiency with the Homecare Company Management System (ClearCare).
- Must be 18 years of age or older.
- CNA license or prior caregiver experience preferred.
- CPR and First Aid certifications preferred.
- Reliable transportation.
- Maintain an established work schedule.
- Effective interpersonal, communication, organizational, and planning skills.
- Ability to maintain confidentiality and establish effective working relationships.
- Must be "Fit for Duty" with a valid driver’s license and automobile insurance.

Schedule
