Patient Care Assistant On Call Home Health

Overview

The On-Call Patient Care Assistant provides personal care, comfort measures, and environmental services for clients, including those needing special precautions. This role requires demonstrating knowledge of growth and development across the lifespan and adhering to departmental policies and procedures. The position also involves promoting a safe work environment and collaborating with the Private Duty team to maintain high-quality client care.

What You Will Do

• Demonstrates accountability for ensuring availability of necessary resources (equipment, instruments, supplies).
• Maintains a clean, safe, and orderly work/patient environment by cleaning work stations/areas, stocking supplies, and placing equipment in proper storage areas.
• Demonstrates competence in following the plan of care to ensure patients receive consistent, competent, and ethical care within legal standards of practice, while maintaining patient privacy and support for their value systems.
• Provides care consistent with verbal and/or written instructions.
• Demonstrates a competent understanding of assigned plans of care within the appropriate scope of practice and responsibilities, as directed by the RN/LPN, including accurately completing all required documentation.
• Participates positively in training and support of all team members, volunteers, and students as needed.

Qualifications

• Education: Program Graduate from a Personal Care Aide Training Program approved by the VA Department of Health/DMAS (Required), or Program Graduate from a Nurse Aide Training Program approved by the VA Board of Nursing (Preferred).
• Experience: Prior work experience in a home care setting (Preferred).
• Licenses and Certifications: CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 days (Required).
• Other: Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job. Valid Drivers License Required.
