Patient Care Aide - PCA

Avenues Home Care of Tennessee serves the Northeast Tennessee area including Bristol, Johnson City, Bluff City, Blountville, Piney Flats, Sturgeon, Oak Grove, and many others. Our CareMatch program assesses and matches clients with the right caregiver to ensure the best care possible, using behavioral assessment tools to align caregiver strengths with client needs.

Responsibilities

  • Assist the client with Activities of Daily Living (ADLs) as specified in the client’s plan of care, including but not limited to:
    • Bathing, dressing, grooming, and other personal hygiene
    • Toileting, transferring, eating, and continence care
  • Provide medication assistance, housekeeping, laundry, meal preparation, feeding, shopping, transportation to appointments, and companionship
  • Perform other household tasks as outlined in the client’s plan of care
  • Report daily activity to your supervisor through the agency management system, daily logs, and other communication methods (phone, email, agency app, or in-person)
  • Practice safety in all client-related activities
  • Lift or push up to 50 pounds
  • Demonstrate honesty, accountability, and integrity
  • Provide superior care and remain vigilant of any changes in client condition
  • Maintain personal appearance in compliance with company policies, including dress code
  • Report for shifts on time and communicate scheduling changes promptly
  • Report suspected abuse or neglect immediately, as required by law
  • Document unusual behaviors, events, conditions, or individuals
  • Maintain confidentiality of records and client information
  • Attend meetings and in-services as requested
  • Abide by all company policies and procedures, including HIPAA regulations
  • Perform other duties as assigned

Requirements

  • Good communication skills and ability to follow verbal and written assignments
  • Proficiency with the Homecare Company Management System—ClearCare
  • Ability to perform essential job functions with or without reasonable accommodations
  • 18 years of age or older
  • Maintain an established work schedule
  • Effective interpersonal, communication, organizational, and planning skills
  • Ability to establish and maintain effective working relationships
  • “Fit for Duty” physical condition, valid driver’s license, and automobile insurance
  • CNA License or prior caregiver experience preferred
  • CPR & First Aid Certifications preferred
  • Reliable transportation

Preferred Experience

  • Prior experience assisting with ADLs or caring for elderly, disabled, and vulnerable clients

Equipment Used

  • Telephone, smartphone, or tablet
  • Personal Protective Equipment (PPE) provided

Safety Expectations

  • Work safely and follow safety rules
  • Report unsafe working conditions and behavior

Schedule

  • Full Time
  • Part Time
  • 8, 10, or 12-hour shifts
  • PRN (as needed)
  • Short shifts available
