Qualifications
A four-year degree in a related field is required; equivalent experience of 4+ years may be considered.
Essential Job Functions
- Works in a team setting as one component of the client's coordinated care.
- Maintains a relationship with the client that fosters trust and understanding distinct from the provider role.
- Serves as a role model that provides reliable information, appropriate referrals, and emotional support.
- Participates in Continuous Quality Improvement (CQI) activities, making recommendations for strategies to improve quality of care.
Competencies
- Takes initiative and handles various tasks simultaneously without direct supervision.
- Knowledgeable in adult learning principles, management leadership, and community resources.
- Compliant with policies, standards, regulations, and protocols.
- Proficient in data bases such as Excel spreadsheets, EMR, etc.
Supportive Services
- Responds to phone calls in a respectful and timely manner.
- Responsible for medication log, sample med list sign out, and providing instructions on how to take issued medications.
- Keeps work areas neat and tidy.
- Faxes records/authorizations in a timely manner.
- Advocates for the client by bringing concerns about services to the attention of relevant parties.
Physical Requirements
- Must have direct client contact to provide program activities.
- Willingness to lift up to 50 pounds.
- Requires sitting, stooping, squatting, standing, and long periods on a computer.
