Patient Access Representative
About the role
Craniometrix is hiring a Patient Access Representative to strengthen non-clinical care support across dementia-care navigation and Medicare GUIDE support for patients, families, caregivers, and providers. This role focuses on patient access service representation within dementia care navigation support, ensuring every handoff shows what is complete, what is pending, and who owns the next step.
Responsibilities
- Own, coordinate, review, maintain, and track the Patient Access Representative workflow at Craniometrix by:
- Documenting status and next action for authorized teammates.
- Maintaining accurate administrative care records.
- Coordinating routine non-clinical follow-up.
- Protecting sensitive information throughout the workflow.
- Routing clinical or policy questions to authorized staff.
- Apply insurance verification and data accuracy in patient access service representation within dementia care navigation support.
- Provide non-clinical support while tracking exceptions, owners, and deadlines tied to patient access service representation.
- Document Medicare GUIDE Administration decisions clearly so another teammate can follow the reasoning.
- Use sound judgment in care coordination, especially when escalation is required instead of assumption.

Pay
