Nature of Work: Pathways to Community is a collaborative partnership between the Coconino County Sheriff’s Office and Coconino County Health and Human Services (CCHHS) that provides reentry support to individuals transitioning from the Coconino County Detention Facility back into the community. The Front Desk Resource Specialist serves as the primary point of contact for clients at the Pathways to Community program, engaging with clients upon release, providing basic resource navigation, and triaging individuals to appropriate staff or community resources based on their needs. This position conducts client screenings, supports the Community Health Worker team, and provides administrative assistance to the Pathways team. The role operates on-site with no remote work opportunities.

This position is funded by Pathways Legislative SB1735 through 06/30/2027.

Responsibilities

Front Desk Resource Navigator I

  • Serves as the primary point of contact for Pathways to Community clients during booking and release from the Coconino County Detention Facility.
  • Conducts screenings to determine client needs and inform individualized reentry plans.
  • Monitors and tracks client progress, adjusting services as needed.
  • Documents all client interactions and service delivery progress per program policy.
  • Works to improve health equity and resource availability by participating in Pathways service teams and case staffings.
  • Conducts outreach for post-release follow-up and serves as a liaison between Pathways, partners, clients, and the community.
  • Collaborates with community partners to provide seamless support services.
  • Maintains accurate records of program activities.
  • Stays informed on best practices and emerging trends in reentry programs.
  • Assesses individual needs and provides support services for successful reintegration.
  • Identifies and engages vulnerable populations.
  • Provides cultural mediation, interpretation, or translation related to health services.
  • Refers clients to health, social services, or other identified needs, assisting with system navigation.
  • Delivers health education and disseminates medical, behavioral, or dental health information to community groups.
  • Maintains updated client records with plans, notes, and related information.
  • Advocates for individual or community health needs with government agencies or service providers.
  • Attends community meetings or health fairs to build relationships and provide resources.
  • Contacts clients to ensure completion of required or recommended actions.
  • Distributes informational materials to vulnerable communities.

Front Desk Resource Navigator II

  • Performs all duties of a Front Desk Resource Navigator I.
  • Provides administrative support to the Pathways Manager, including case staffing management and chart reviews.
  • Conducts outreach to community resources to partner in client trainings and support.
  • Provides resource navigation for self-directed client re-entry and disseminates health information to targeted groups.
  • Develops additional trainings as required for served populations.
  • Completes quarterly file reviews and post-screening report audits.

Requirements

Front Desk Resource Navigator I

  • Two years of experience in a directly related role serving community-based or vulnerable populations; OR any equivalent combination of education, training, and experience.
  • Must obtain an Arizona driver’s license by date of hire and maintain it throughout employment.
  • Final job offer contingent upon a satisfactory 5-year Arizona Motor Vehicle Report.
  • Fingerprint clearance may be required.

Front Desk Resource Navigator II

  • Three years of experience in a directly related role serving community-based or vulnerable populations; OR any equivalent combination of education, training, and experience.
  • Must obtain an Arizona driver’s license by date of hire and maintain it throughout employment.
  • Final job offer contingent upon a satisfactory 5-year Arizona Motor Vehicle Report.
  • Fingerprint clearance may be required.

Qualifications

The ideal candidate will have a deep connection to justice-involved clients, with lived experience that fosters client engagement. They will be committed to supporting successful reintegration, familiar with challenges faced by individuals leaving incarceration, and enthusiastic about maintaining a positive, welcoming environment. The candidate should be adept at navigating services, assisting with basic needs, and providing front desk administrative support.

This position is safety-sensitive and non-exempt from overtime.

Skills

  • Working knowledge of challenges faced by reentering individuals and effective program development.
  • Familiarity with programs and services from governmental and non-governmental organizations improving community health.
  • Understanding of basic public health issues, methods, and principles.
  • Knowledge of criminal justice systems, substance use disorders, and social determinants of health.
  • Skills in interpersonal communication, community building, and conflict mediation.
  • Ability to deliver trainings and provide individual coaching.
  • Proficiency in Microsoft Office applications and web-based technologies.
  • Effective communication with individuals from diverse cultural and socioeconomic backgrounds.
  • Ability to supervise and motivate clients, follow instructions, and work independently.
  • Behavior management strategies and assessment skills.
  • Adaptability to individual client needs and confidentiality maintenance.
  • Ability to work with clients with behavioral, emotional, learning, or physical disabilities.
  • Competence in managing competing demands and working impartially with diverse populations.
  • Collaborative teamwork and initiative in rapidly changing environments.
  • Commitment to workplace safety and effective working relationships with employees, agencies, and the public.
