About the Role

We are seeking Wound Care Nurse Practitioners to provide specialized wound care services to patients living in long-term care facilities.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with Kentucky law.
• Order, review, and follow up on treatments and diagnostic testing.
• Communicate regularly with nursing home staff regarding changes in patient condition and care plans.
• Collaborate with primary care physicians.

Requirements

• Current, unrestricted Nurse Practitioner license in the state of Kentucky.
• Board Certification as a Nurse Practitioner.
• DEA licensure (optional).
• Clinical background in adult, family, or geriatric specialties.

Qualifications

• Clinical experience in wound care, including chronic, debilitating, stalled, or infected wounds.
• Comfort diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds, as well as Kennedy Ulcers.
• Ability to stage pressure injuries according to the NPIAP guidelines.
• Familiarity with wound care best practices and advanced wound care products (e.g., collagens, alginates, foams, silvers).
• Experience with wound vacs and other durable medical equipment (DME), such as pressure-relieving or reducing devices.
• Strong preference for candidates with wound certification or willingness to obtain certification.
• Experience working in a long-term care setting.
• Ability to work with an Electronic Medical Record (EMR) system.
• Ability to work within an interdisciplinary care team to coordinate patient care.
