About the Role
CareMed Health Solutions is a facility-based team of medical providers who deliver medical services to patients living in long-term care facilities. We aim to provide quality care to residents while collaborating with the facility to ensure the best possible outcomes. As a Wound Care Nurse Practitioner, you will have the opportunity to positively impact the lives and well-being of the elderly population by providing specialized wound care services in skilled nursing facilities.
Responsibilities
- Provide wound care services for long-term care patients residing in skilled nursing facilities.
- Prescribe appropriate medications, including controlled substances in accordance with Kentucky Law.
- Order, review, and/or follow up with treatments and diagnostic testing.
- Communicate regularly and as needed with nursing home staff to discuss changes in patient condition and/or the care plan.
- Collaborate with the primary care physician.
Requirements
- Current, unrestricted Nurse Practitioner license in the state of Kentucky.
- Board Certification.
- DEA licensure (optional).
- Clinical background in adult, family, or geriatric specialties.
- Clinical experience in wound care, including but not limited to chronic, debilitating, stalled, or infected wounds.
- Comfort diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
- Ability to stage pressure injuries according to the NPIAP.
- Familiarity with wound care best practices and advanced wound care products such as collagens, alginates, foams, silvers, etc.
- Experience with wound vacs and other durable medical equipment (DME), such as pressure-relieving/reducing devices.
- Strong preference for candidates with wound certification or willingness to obtain certification.
- Experience working in a long-term care setting.
- Ability to work with an Electronic Medical Record (EMR) system.
- Ability to work within an interdisciplinary care team to coordinate patient care.
