Part-Time Virtual Family Practice Nurse Practitioner - Remote (California Licensed)

About the Role

We're looking for part-time, dynamic Nurse Practitioners who are ready to revolutionize healthcare delivery by conducting virtual wellness visits directly to patients in the comfort of their homes. As a Nurse Practitioner with Sprinter Health, you'll leverage your medical expertise to offer a wide range of healthcare services that could include but not limited to virtual adult and/or pediatric wellness visits, health assessments, and more!

Responsibilities

• Review daily case load and prepare technology to service patients
• Navigate through daily schedule using modern technology and partner with clinical in-home team members (Sprinters)
• Build warm and welcoming connections with diverse patients
• Conduct thorough health risk assessments, medication reviews, and cognitive screenings
• Provide educational information to patients regarding their health and well-being
• Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs
• Provide comprehensive care coordination, including preventive care interventions, medication management, referrals, and documentation

Requirements

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in California
• Credentialed Medicaid provider for the state of California (Medi-Cal)
• Exhibit professionalism, integrity, accountability, confidentiality, care, and compassion
• Willingness to work in a revolutionary environment with last-minute problem-solving
• Technologically savvy with laptops, mobile devices, and HIPAA secure messaging apps
• Strong written and verbal communication skills
• Ability to work independently or in a team environment
• Pass national background check and valid clinical license search
