About the role

Part-time Social Worker - Hospice serving Saratoga, Schenectady, and Montgomery County regions. Day shift. Join our compassionate hospice team providing care in patients’ homes and nursing facilities, supporting individuals and caregivers navigating serious illness and end-of-life care. As a vital member of the interdisciplinary team, you will address emotional, psychological, and social needs while collaborating with nurses, physicians, chaplains, aides, and other healthcare professionals.

Responsibilities

• Provide psychosocial assessments and ongoing emotional support to patients and families.
• Offer counseling and therapeutic interventions related to end-of-life issues, grief, loss, and adjustment.
• Identify barriers that may impact patient well-being and quality of life.
• Connect patients and families with community resources and supportive services.
• Advocate for patients and help ensure care is provided in the most appropriate setting.
• Collaborate with the interdisciplinary team to develop and implement patient-centered care plans.
• Support staff through consultation and education related to psychosocial aspects of hospice care.

Requirements

• NYS Social Work Licensure.
• Valid NYS Driver’s license.
• Minimum 2‑3 years of experience, ideally caring for seniors or those with advanced illness.

The Community Hospice provides a thorough orientation and a supportive work environment.

Pay

Pay range: $30.00 – $43.08 per hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
