About the Role

Support patients with End Stage Renal Disease in an outpatient setting by building meaningful relationships, advocating for patient rights, and assisting in managing physical, mental, emotional, and financial challenges related to dialysis.

Requirements

• Master’s degree in Social Work (MSW) with clinical specialization.
• Licensure per state regulations.
• Knowledge of insurance programs.
• Proficiency in MS Office and DaVita-specific applications within 60 days.
