About the role
The Nurse Navigator leads patient care coordination, providing support, mentorship, and guidance to direct care providers.
Responsibilities
- Assess patient needs and recommend care plans based on severity and skill level
- Ensure timely and safe care delivery
- Coordinate discharge planning
- Manage length of stay and utilization
- Perform pre-op education
- Conduct daily rounds and follow-up care
- Support quality patient care and promote safety
- Ensure regulatory compliance
- Facilitate seamless care models through collaboration and performance optimization
Requirements
- Graduate of an approved nursing school
- Current RN license
- 2-3 years of experience as a registered nurse
- BLS (Basic Life Support) certification
Skills & Physical Requirements
- Ability to lift 50 pounds
- Stand and walk for extended periods
- Perform physical tasks such as reaching, bending, and kneeling
Preferred Qualifications
- BSN (Bachelor of Science in Nursing)
- ACLS (Advanced Cardiovascular Life Support) certification
