Available Shifts
- 7:00am-3:00pm, 3:00pm-11:00pm, and 11:00pm-7:00am
- 2 days a week and every other weekend (holidays as needed)
- Shift differential: 2nd Shift (3pm–11pm): $3.00; 3rd Shift (11pm–7am): $2.00; Weekends: additional $1.00 + shift differential
About the Role
We are seeking a passionate and dedicated Licensed Practical Nurse who is committed to join our team and provide exceptional and compassionate care to residents in a long-term care/skilled nursing environment. This role will be responsible for delivering high-quality nursing care, administering medications, monitoring resident conditions, and supporting the daily clinical needs of our residents.
Responsibilities
- Provide direct nursing care to residents under the supervision of Unit Managers and Director of Nursing within the scope of RN practice
- Administer medications and treatments accurately and in accordance with facility policy
- Participate in admission, discharge, and transfer processes
- Work in partnership with nursing Admins team, physicians, therapists, CNAs, and other team members to deliver comprehensive care
- Perform resident assessments and promptly report changes in condition to the supervising nurse or provider
- Document all care, assessments, treatments, and observations in accordance with regulatory and facility standards in the Point Click Care system
- Assist with care planning and ensure care is delivered according to individualized plans
- Provide wound care and other nursing procedures as needed
- Maintain infection control standards and promote a clean, safe, and respectful environment
- Communicate professionally with residents, families, and staff while showing patience and empathy
- Support and guide LPNs and CNAs to ensure quality resident care and adherence to policies
