About the Role

Senior LIFE is an innovative home and community-based Medicare and Medicaid-funded program which provides all-inclusive healthcare services and support to seniors living in the community. The Physical Therapist will develop, implement, and assess therapy-based initiatives in alignment with individualized and group care plans, requiring direct care, strategic thinking, and the ability to inspire others.

Responsibilities

  • Evaluate members’ mobility levels and specific PT needs, devising comprehensive approaches to increasing strength, flexibility, range of motion, motor control, neuromuscular reeducation, and pain reduction.
  • Develop innovative treatment programs, set goals for members and caregivers, and establish periodic check-ins to gauge progress.
  • Evaluate members’ home environments to minimize hazards and provide additional in-home support as needed.
  • Document all changes in care plans, therapeutic recommendations, and participation in therapies and group activities in Data Pace.
  • Assess members’ functional status and adjust or enhance therapeutic approaches as needed.
  • Identify areas of potential improvement for individual members and the LIFE Center community, then develop and implement these changes.
  • Complete fall risk assessments and develop individual plans to reduce vulnerabilities.
  • Act as an active stakeholder on interdisciplinary teams, collaborating with physicians, nurses, dieticians, occupational therapists, and other caregivers to provide updates, recommendations, and timelines for ongoing Plans of Care.

Requirements

  • Licensed to practice physical therapy in Pennsylvania.
  • Relevant degree from an accredited college or university.
  • At least one year of experience working specifically with an elderly/geriatric population.
  • Valid Pennsylvania driver’s license.
  • Strong clinical competency and ability to troubleshoot diverse circumstances.
  • Ability to work well in a team environment, strong communication skills, and versatility.
  • Genuine desire to aid an elderly community.
