About the Role
Specialize in therapy and rehabilitation within a home health setting, providing essential physical therapy services to patients in their homes.
Responsibilities
- Provide physical therapy services, assess patient needs, and make clinical judgments.
- Develop and implement individualized treatment plans.
- Effectively communicate with patients, families, physicians, and colleagues.
- Travel to various locations to deliver care.
- Handle on-call duties once a month.
- Prioritize tasks, demonstrate attention to detail, and resolve situations with appropriate solutions.
Qualifications & Skills
- Minimum one year of physical therapy experience in an acute setting.
- Valid state Physical Therapy license.
- Excellent communication skills.
- Valid driver’s license with auto insurance.
- Preferred: Home care experience.
- Disciplined work ethic, observant, and timely in completing tasks.
