Role Overview
The Pharmacist provides safe, effective, and cost-efficient pharmaceutical care, developing and monitoring evidence-based pharmacotherapy plans focused on patient outcomes and quality of life. They assess medications for indication, safety, effectiveness, and adherence, while reviewing lab results and documentation.
Key Responsibilities
- Deliver pharmaceutical care tailored to specific patient care areas.
- Interpret and clarify physicians' medication orders, ensuring appropriate therapy.
- Review medication profiles to prevent drug interactions, contraindications, and duplications.
- Consult with medical staff on drug therapy issues and monitor patient lab results.
- Provide drug information to healthcare professionals and discharge medication counseling.
- Calculate dosages, verify orders, and prepare compounded and sterile medications.
- Supervise pharmacy technicians, students, and interns, ensuring accuracy and adherence to standards.
- Collaborate with medical and nursing teams to resolve pharmacy-related issues.
- Assist in updating departmental policies and coordinate pharmacy activities.
Qualifications & Skills
- Bachelor's Degree in Pharmacy (required).
- Master's Degree preferred.
- Strong clinical knowledge, communication, organizational, and supervisory skills are essential.
