About the role

This role involves providing comprehensive psychosocial assessments and social work services to support members in managing their healthcare. You will develop patient-centered, cost-effective care plans, utilize diverse intervention strategies, and promote multidisciplinary collaboration that respects individual cultural, religious, and ethnic differences.

Maintain thorough knowledge of healthcare benefits, community resources, and relevant laws, ensuring proper documentation and contributing to program development and patient education activities.

Responsibilities

• Conduct comprehensive psychosocial assessments for members.
• Develop patient-centered, cost-effective care plans.
• Utilize diverse intervention strategies to support members.
• Promote multidisciplinary collaboration with respect to cultural, religious, and ethnic differences.
• Maintain up-to-date knowledge of healthcare benefits, community resources, and relevant laws.
• Ensure proper documentation of all social work services.
• Contribute to program development and patient education activities.

Requirements

• Master’s degree in social work from an accredited institution.
• Minimum of one year of relevant experience or completion of an internship.
• At least three years of related experience.
• State-specific licensing requirements; must obtain required credentials within six months of hire.
• Basic Life Support (BLS) certification.
• National Provider Identifier (NPI).

Skills

• Effective teamwork and leadership abilities.
• Strong knowledge of healthcare resources and community support systems.

Preferred Qualifications

• Advanced experience in social work or healthcare settings.
• Licensure as a Licensed Clinical Social Worker (LCSW).
