Part-Time Hospice Palliative Care Nurse

About the Role

The Hospice/Home-Based Palliative Care Medical Social Worker provides psychosocial support and social services to patients in community and facility settings, primarily focusing on end-of-life care.

Responsibilities

• Conducts assessments of patients' social, emotional, and safety needs.
• Develops individualized care plans utilizing community resources.
• Collaborates with interdisciplinary teams to coordinate care.
• Ensures compliance with policies and regulations.
• Participates in quality improvement activities.
• Maintains current knowledge of social work practices and resources.

Requirements

• Master’s degree in social work.
• Relevant licensure.
• Experience in elder or healthcare settings.
• Strong assessment, counseling, and communication skills.
• Knowledge of palliative care principles, medical terminology, and community resources.
• Ability to operate clinical documentation tools.
• Ability to travel to patient homes.

Preferred Experience

• Previous hospice or home care experience.
• Interdisciplinary team collaboration.
• Familiarity with managed care and regulatory requirements.
• Strong organizational, problem-solving, and customer service skills.
