About the Role
The Hospice/Home-Based Palliative Care Medical Social Worker provides psychosocial support and social services to patients in community and facility settings, primarily focusing on end-of-life care.
Responsibilities
- Conducts assessments of patients' social, emotional, and safety needs.
- Develops individualized care plans utilizing community resources.
- Collaborates with interdisciplinary teams to coordinate care.
- Ensures compliance with policies and regulations.
- Participates in quality improvement activities.
- Maintains current knowledge of social work practices and resources.
Requirements
- Master’s degree in social work.
- Relevant licensure.
- Experience in elder or healthcare settings.
- Strong assessment, counseling, and communication skills.
- Knowledge of palliative care principles, medical terminology, and community resources.
- Ability to operate clinical documentation tools.
- Ability to travel to patient homes.
Preferred Experience
- Previous hospice or home care experience.
- Interdisciplinary team collaboration.
- Familiarity with managed care and regulatory requirements.
- Strong organizational, problem-solving, and customer service skills.
