Part Time Direct Support Professional

About the role

Horizon Health and Wellness, Inc. seeks caring individuals to join our team as Direct Support Professionals. This role provides care services, community integration guidance, and consultation to individuals with intellectual and developmental disabilities. The schedule is Monday through Friday from 12 PM - 5 PM, with the possibility to pick up additional part-time shifts.

Responsibilities

• Provide care services to individuals with intellectual and developmental disabilities.
• Offer community integration/inclusion guidance and consultation to individuals, families, and significant involved persons.
• Develop opportunities for persons with disabilities to participate in community activities while ensuring their health, safety, and well-being.
• Ensure the health, safety, and well-being of residential participants living in the facility through proper supervision.

Requirements

• High School diploma or GED.
• Must be 21 years of age or older.
• Possess valid driver’s license with acceptable driving record, MVD report, and reliable transportation.
• Ability to prepare required paperwork to support and document activities in the contractually required timelines.
• Ability to type and have working knowledge/familiarity with computers and electronic clinical record keeping.
• Successfully complete certification and trainings for First Aid, CPR, Article 9, Medication, and Prevention/Support, Direct Care Worker, and any other required training.
• Able to operate independently with minimal oversight.
• Demonstrated skills to ensure respectful communication to participants and coworkers.
• Must obtain and maintain a fingerprint clearance card and pass a background check along with Central Registry Clearance.
• Willingness to work nights, weekends, holidays, and a varied schedule each week.

Qualifications

• Qualifications Required: High School diploma or GED, must be 21 years of age or older, valid driver’s license with acceptable driving record, MVD report, and reliable transportation, ability to prepare required paperwork, typing and computer familiarity, successful completion of relevant certifications and trainings, ability to operate independently, demonstrated respect in communication, fingerprint clearance and background check.
• Qualifications Preferred: Bilingual English/Spanish or English/Sign Language, two years of experience in a person-centered community integration program, two years of experience in habilitation/rehabilitative assistance to persons with psychiatric and/or developmental disabilities, very strong interpersonal skills.
