Part Time Direct Support Professional

About the role

Horizon Health and Wellness, Inc. seeks caring individuals to join our team as Direct Support Professionals. This role provides care services to individuals with intellectual and developmental disabilities, community integration/inclusion guidance, and consultation to individuals, families, and significant involved persons.

Responsibilities

• Provide care services to individuals with intellectual and developmental disabilities.
• Provide community integration/inclusion guidance and consultation to individuals, families, and significant involved persons.
• Develop opportunities for persons with disabilities to participate in community activities while ensuring the health, safety, and well-being of residential participants living in the facility through proper supervision.

Requirements

• High School diploma or GED.
• Must be 21 years of age or older.
• Possess valid driver’s license with acceptable driving record, MVD report, and reliable transportation.
• Ability to prepare required paperwork to support and document activities in the contractually required timelines.
• Ability to type and have working knowledge/familiarity with computers and electronic clinical record keeping.
• Ability to successfully complete certification and trainings for First Aid, CPR, Article 9, Medication, and Prevention/Support, Direct Care Worker and any other required training.
• Able to operate independently with minimal oversight.
• Demonstrated skills to ensure respectful communication to participants and coworkers.
• Must obtain and maintain a fingerprint clearance card and pass a background check along with Central Registry Clearance.
• Willingness to work nights, weekends, holidays, and a varied schedule each week.

Skills

• Bilingual English/Spanish or English/Sign Language (preferred).
• Prior experience in Habilitation or Rehabilitation (preferred).
• Two years of experience in a person-centered community integration program assisting persons with developmental and/or psychiatric disabilities (preferred).
• Two years of experience in habilitation/rehabilitative assistance to persons with psychiatric and/or developmental disabilities (preferred).
• Very strong interpersonal skills (preferred).

Schedule

The schedule for this position is Wednesday and Friday from 3 PM - 8 PM (10 hours total). There will be the opportunity to pick up extra part time shifts when needed by the agency.
