Part Time Direct Support Professional

About the role

Horizon Health and Wellness, Inc. seeks compassionate, reliable individuals interested in supporting others on an as-needed basis. This role involves providing essential care and support services to individuals with intellectual and developmental disabilities.

Responsibilities

• Provide essential care and support services to individuals with intellectual and developmental disabilities.
• Assist with community integration and inclusion, offering guidance to individuals and their support networks.
• Create meaningful opportunities for participation in community activities.
• Ensure the health, safety, and overall well-being of individuals living in our residential setting by providing proper supervision and support.

Requirements

• High School diploma or GED
• Must be 21 years of age or older
• Possess valid driver’s license with acceptable driving record, MVD report and reliable transportation
• Able to prepare required paperwork to support and document activities in the contractually required timelines
• Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping
• Able to operate independently with minimal oversight
• Demonstrated skills to ensure respectful communication to participants and coworkers
• Must obtain and maintain a fingerprint clearance card and pass a background check along with Central Registry Clearance
• Willingness to work nights, weekends, holidays and a varied schedule each week

Qualifications

• Bilingual English/Spanish or English/Sign Language (preferred)
• Prior experience in Habilitation or Rehabilitation (preferred)
• Two years of experience in a person-centered community integration program assisting persons with developmental and/or psychiatric disabilities (preferred)
• Two years of experience in habilitation/rehabilitative assistance to persons with psychiatric and/or developmental disabilities (preferred)
• Very strong interpersonal skills (preferred)
