Part Time Case Manager

About the role

The Case Manager at the RESET Center provides person-centered, trauma-informed care coordination, engagement, and discharge planning for individuals in a sobering program. Working collaboratively with community partners, law enforcement, and healthcare providers, they facilitate rapid discharges and seamless linkage to community resources.

Responsibilities

• Support participants with empathy, respect, and motivational interviewing techniques.
• Conduct brief assessments, crisis de-escalation, and develop short-term discharge plans.
• Coordinate referrals to substance use treatment, mental health services, shelters, and emergency care.
• Support crisis intervention, document services accurately, and maintain collaborative relationships with community agencies.
• Complete required training and perform additional duties as assigned.

Qualifications

• 1-3 years of experience in harm reduction, crisis services, or behavioral health.
• High School diploma or equivalent; experience supporting law enforcement diversion preferred.

Skills

• Ability to obtain Overdose Prevention and Narcan training, Basic Life Support (BLS), and demonstrate computer literacy.
• Strong attention to detail, timely documentation, and trauma-informed approach.
• Valued qualifications include experience with Managed Alcohol Programs, an associate degree, or lived experience related to substance use or homelessness.

Pay
