Job Responsibilities
- Clinically assesses patient and family needs and coordinates, advocates, and problem-solves to enhance their functioning.
- Completes psychosocial assessments, gathering social, psychosocial, cultural, and financial information to determine the level of support the family requires.
- Develops plans based on assessments and assists families in implementing these plans (including referrals, follow-up calls, and DCF reporting).
- Participates in interdisciplinary teams, accepting “warm hand-offs” from primary care providers and referring to appropriate services within the agency and the broader community.
- Attends clinical Team Meetings and Component Meetings as assigned, and seeks out consultation or supervision as needed.
- Maintains compliance with CHC standards and policies as outlined in annual performance appraisals.
Administrative
- Completes required documentation by state regulations, Joint Commission standards, and CHC policies in a timely manner.
- Locks WHO notes no later than the end of the next business day after completion.
- Completes and submits reports as required for external agencies such as DCF, Family Relations, probation, and others, with appropriate client releases signed.
- Participates in agency-wide committees as assigned.
Required Experience & Qualifications
- Master's Social Work degree (MSW) required; Licensed Clinical Social Worker (LCSW) preferred.
- Minimum of two years of related experience working with children and families.
- Written and oral bilingual fluency in Spanish is required.
Physical Demands & Environment
- Combination of sustained periods of sitting at a keyboard, walking, and stair climbing.
- Minimal lifting and carrying activities.
- Must be available to work one evening a week and one Saturday per rotation.
Communications Skills
- Excellent oral and written communication skills are required.
- This position interfaces with patients, family members, community agencies, and other providers within the organization.
- Confidentiality of behavioral health information is strictly maintained.
- Responsible for client data entry, accessing medical system information, and maintaining confidentiality of patient and business information according to CHC policies.
