Palliative Outpatient Social Work
SUMMARY OF POSITION
The Palliative Care Social Worker, a critical member of the Care Transitions team, is responsible to assess patient's social and behavioral needs, provide focused support to patients to address health inequities and aims to address the social, emotional, and environmental factors impacting their well-being. The Social Worker collaborates closely with the Palliative Care team including physicians, nurses, chaplains, community health workers and the inpatient Palliative Care team to enhance patient outcomes, reduce readmissions, and improve the overall patient experience.
MINIMUM QUALIFICATION(S)
- Master's degree in Social Work (MSW)
- Prior Palliative Social Work Experience
- Current Social work licensure in Pennsylvania
- Minimum of two (2) years experience
- Previous experience with Cerner and/or Wellsky
- Certification in case management from a nationally recognized organization
