Qualifications
- Board Certified or Board Eligible in Hospice and Palliative Care (fellowship trained with Palliative Care experience is highly preferred)
- Exceptional clinical and communication skills
- Ability to obtain a Georgia Medical License
- EPIC experience a plus
Responsibilities
- Perform inpatient and palliative care consultations
- Provide clinical leadership and expertise within the palliative care clinical team
- Assist patients and families in navigating complex medical decisions
- Manage physical and emotional symptoms of patients with serious and complex illnesses
- Meet with families and patients to determine goals of care and document patients advance care wishes
- Proactively identify opportunities to improve the patient and family experience of care
