Palliative Care Navigator (LPN)

About the role

The Palliative Care Navigator at Agape Care Group provides services to patients and their families navigating the healthcare continuum. This includes ensuring patients have a voice in their care and promoting informed decision-making. The role involves working directly with patients and their legal representatives, as well as educating internal and external key stakeholders.

Responsibilities

• Work directly with patients and their legal representatives to ensure they have a voice in their care and information to promote informed decision making.
• Serve as an educator to internal and external key stakeholders.
• Under the direction and supervision of the registered nurse, conduct patient visits and provide routine care.
• Communicate with the care team, the patient, and their family.

Requirements

• Graduate of an accredited program for Practical Nurse or Vocational Nurse.
• Minimum 3 years of experience required in General Nursing or Medical Assistance with current home health, hospice, medical, surgical or critical care experience preferred.
• Reliable transportation. Valid Drivers License and Insurance.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: Graduate of an accredited program for Practical Nurse or Vocational Nurse.
