Primary Accountability
The PACE Driver is under the supervision of the Center Manager and/or Transportation Coordinator, responsible for the direct transportation of FHCN PACE participants to and from their destinations. The position involves constant interaction with caregivers and community agencies.
Description Of Primary Responsibilities
- Effectively communicates with participants, families, staff, and other agencies and departments.
- Drives a specially equipped van to deliver physically limited persons to varied destinations.
- Affirms that wheelchair lifts and tie-down equipment are properly operated and that participants are delivered to appointments in a timely manner.
- Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery route.
- Affords assistance to participants on and off the van in a safe and courteous manner.
- Deals with medical equipment and other emergencies that may arise.
- Inspects the van and maintains the vehicle in a clean, orderly, and safe condition. Reports on the need for maintenance.
- Ensures all necessary program records are maintained according to policy.
- Observes each participant for any change in physical, mental, emotional, and social functioning and shall report such changes to the licensed nurse.
- Completes documentation as required.
- Affords assistance in the coordination of 24-hour care delivery with support from the FHCN Navigation Center.
Description Of Primary Attributes
- Professional & Technical Knowledge: Must possess work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, and technical skills.
- Experience: At least two years of experience transporting individuals with special mobility needs.
- Interpersonal Skills: Interacts in a friendly and professional manner with participants and their families. Communicates effectively and openly with other Interdisciplinary Team members on patient status and needs.
- Medical Clearance: Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
- Licenses & Certifications: Valid CA driver’s license and an acceptable driving record are required. Upon employment, a defensive driving course, first aid, and CPR certifications are required to be kept up to date.
- Physical Demands: Significant lifting, carrying, pushing, and/or pulling, stooping, grasping, kneeling, crouching, standing, bending, walking, sitting, using arms and legs during transit escort, and assisting with lifting participants and helping them turn, walk, or stand.
- Work Environment: Employees must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the FHCN PACE members’ care. On occasion, the Employee will spend time in a vehicle transporting participants. The primary work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled office temperatures.

- Min Salary Rate: $21.00
- Max Salary Rate: $28.60
