About the role
The PACE Home Caregiver under the direction and supervision of the Home Care Supervisor provides personal care, light housekeeping, and/or supervision to North East Medical Services PACE participants in their home. The role involves assisting participants with tasks outlined in their individualized care plans, reporting changes in their condition, and participating in social and recreational activities.
Responsibilities
- Provides assistance with personal care including tasks such as peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation.
- Performs housekeeping tasks such as changing linens, laundry, and light cleaning.
- Supervises and offers emotional support to participants as needed.
- Reports observed changes in participants' conditions to the Home Care Supervisor and other PACE clinical staff.
- Observes, tracks, and reports participant status, including vital signs and nutritional intake.
- Communicates participant health and safety concerns to the Home Care Supervisor and other program staff.
- Demonstrates safe practices using proper body mechanics and adhering to universal precautions.
- Interacts in a friendly, courteous, and professional manner with participants and their families/caregivers.
- Participates in social and recreational activities with participants as appropriate.
- Utilizes Epic appropriately and efficiently, serving as a content and workflow expert for resolving issues and proposing solutions.
- Performs other job duties as required by manager/supervisor.
Qualifications
- Must complete and pass the North East Medical Services PACE training curriculum upon hire.
- Current BLS and First Aid required.
- Ability to communicate effectively.
- Minimum 1 year of experience as a Personal Care Aide or EMT, or certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
- Knowledge of the geriatric population.
- Knowledge of safe and sanitary personal care techniques for elderly population with physical limitations.
- Knowledge of proper body mechanics during provision of assistance with Activities of Daily Living.
- Home Health Aid certification.
- Certified Nursing Assistant (CNA) certification.
- One year experience working with the frail or elderly population.
- Fluency in English and Cantonese. Fluency in other languages is an asset.
