About the role

Humboldt Park Health is a 200-bed, acute care facility deeply committed to serving the Humboldt Park community since 1894. We provide patient-centered healthcare focusing on quality of care, from everyday care to critical treatments.

Responsibilities

• Reviews, processes, and modifies medication orders to maintain an accurate medication profile.
• Supervises, directs, and reviews activities involving compounding, storing, inventory, labeling, recording, or preparing medications by pharmacy technicians.
• Maintains drug therapies by evaluating appropriateness of use, dose, dosage form, regimen, route, therapeutic levels, drug interactions, and therapeutic duplication.
• Evaluates drug use for compliance with prescribing criteria, guidelines, formulary, and protocols established by Norwegian American Hospital.
• Detects, monitors, documents, and reports adverse drug reactions and medication errors.
• Serves as a resource for drug information, including drug identification, side effects, adverse events, interactions, medication counseling, etc., for all practitioners and patients.
• Involves in QA/PI projects and initiatives to assure proper control, documentation, and record keeping of controlled substances.
• Maintains compliance with state and federal regulatory requirements.

Requirements

• Graduate of an ACPE accredited college of pharmacy.
• Pharmacist licensure required.
