About the role

At Grafton Integrated Health Network, we provide direct, person-centered care in a structured residential setting, supporting individuals with complex needs in developing independence, life skills, and emotional well-being.

Responsibilities

  • Delivers accurate medication administration and ensures client safety.
  • Supports client health and hygiene, adheres to infection control procedures.
  • Conducts and records 15-minute health and safety checks after bedtime.
  • Provides supervision, safety, and crisis management for the client population.
  • Documents client behavior and participation in programming, adheres to scheduled activities.
  • Role models positive, mature behavior and utilizes proactive behavioral strategies.
  • Reports serious incidents, participates in reviews, and implements follow-up measures.
  • Knows and promotes clients' achievement of behavioral goals through behavior strategies and teaching alternative behaviors.
  • Instructs assigned clients to achieve education/residential goals and objectives or ID Waiver outcomes.
  • Captures and documents behavioral and instructional data accurately and reliably.
  • Maintains a high level of engagement and monitoring with clients.
  • Implements planned activities throughout the workday.
  • Builds trusting, respectful relationships with clients and helps them do the same.
  • Demonstrates knowledge about the symptoms and supports associated with diagnoses of assigned clients and a trauma-informed care approach.
  • Manages behavioral crises safely.
  • Collaborates with the Treatment Team to provide continuity of support and care.
  • Exhibits exemplary customer service with guardians, agency personnel, and visitors.
  • Safeguards and maintains a safe residence/classroom environment.
  • Affirms good nutrition and, in group homes, assists with meal preparation.
  • Provides transportation/community mobility assistance.
  • Assists with purchases and is accountable for program and client money.

Requirements

  • High School Diploma and six months of direct care experience with children and/or adolescents.
  • Associates degree and three months of direct care experience with children and/or adolescents.
  • Bachelor’s degree in Human Services field.
  • Valid driver's license with an acceptable driving record.
  • Demonstrates moderate computer skills - logging in, navigating the web for information, starting and saving documents in Microsoft, and basic keyboarding.

Qualifications

  • Education: High School Diploma or Associates degree or Bachelor’s degree in Human Services field.
  • Experience: Six months of direct care experience with children and/or adolescents or three months of experience with a Bachelor’s degree.
  • License: Valid driver's license with an acceptable driving record.
  • Skills: Moderate computer skills, including logging in, navigating the web for information, starting and saving documents in Microsoft, and basic keyboarding.
