Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. About the Role

The Social Work Case Manager works creatively and effectively in the community to engage individuals living on the street through outreach by utilizing a harm reduction approach. The position serves as a case manager for adults clinically complex adults experiencing street homelessness, with the goal of placement into transitional and permanent housing. The position works collaboratively across teams and organizations to achieve this goal.

Responsibilities

• Engage individuals living on the street through outreach using a harm reduction approach
• Assess and document client needs and develop individualized service plans
• Collaborate with other agencies and organizations to secure housing placements for clients
• Provide crisis intervention and support services to clients
• Conduct intake assessments and ongoing case management for adults experiencing street homelessness
• Work collaboratively with team members to ensure comprehensive care for clients

Requirements

• Bachelor's degree in Social Work or related field
• Current LCSW license or eligibility to obtain
• Experience working with adults experiencing street homelessness
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Knowledge of trauma-informed care principles

Qualifications

• Master's degree in Social Work or related field
• Experience in a supervisory capacity
• Experience with interagency collaboration

$56,000 - $61,000
