About the opportunity

Detroit Recovery Project is seeking a compassionate and mission-driven Outreach Nurse Practitioner to join our Integrated Health team on a part-time basis. This position plays a critical role in delivering healthcare services directly to individuals experiencing homelessness and other vulnerable populations throughout the community.

What We're Looking For

We are seeking an experienced Nurse Practitioner who is passionate about serving vulnerable populations and committed to reducing barriers to healthcare access. The ideal candidate has experience working with individuals experiencing homelessness, substance use disorders, behavioral health concerns, and other complex healthcare needs. Candidates should be comfortable working in community-based settings and collaborating within a multidisciplinary team.

What You'll Do

  • Provide primary healthcare services through community-based outreach efforts
  • Establish trusting relationships with participants while providing support, encouragement, and health education
  • Conduct health assessments and develop treatment recommendations
  • Deliver trauma-informed and harm-reduction-focused care
  • Conduct STI assessment, testing, treatment, and follow-up services
  • Provide vaccinations, wound care, point-of-care testing, and health education
  • Assess and counsel individuals regarding substance use disorders and medication-assisted treatment options
  • Provide birth control and pregnancy options counseling
  • Aid participants in navigating healthcare, behavioral health, housing, and community resources
  • Cover referrals and facilitate connections to care
  • Document services and encounters within the electronic health record
  • Respond appropriately to medical emergencies within scope of practice
  • Participate in mobile outreach activities and community engagement efforts
  • Assist with loading and unloading outreach and medical supplies as needed
  • Provide occasional coverage on DRP's mobile medical units
  • Support integrated care initiatives in collaboration with medical, behavioral health, and peer support teams

Scheduling Expectations

This position is scheduled for up to 25 hours per week. Coverage on Wednesdays is required. Additional hours and scheduling will be determined based on program needs and candidate availability.

Qualifications

  • Current Michigan Registered Nurse (RN) license required
  • Current certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) in good standing
  • Minimum of two (2) years of experience working with substance use disorder, behavioral health, homeless, or underserved populations preferred
  • Experience providing community-based or outreach services strongly preferred
  • Understanding of trauma-informed care and harm reduction principles
  • Familiarity with Detroit-area resources and service providers preferred
  • Valid Michigan Driver's License and reliable transportation required
  • Strong communication, organizational, and relationship-building skills
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Proficiency with Microsoft Office Suite
  • Experience with Electronic Medical Records/Electronic Health Records preferred
  • Experience with NextGen EHR preferred
