About the Role

Be the Bridge to Stability and transform lives through housing at Riverbend Community Mental Health, a trusted leader in New Hampshire’s behavioral health community. This role is an opportunity to be on the front lines of ending homelessness and advancing recovery for adults living with serious and persistent mental illness. As an Outreach Counselor – Housing Bridge, you will help individuals secure and sustain safe, stable, and meaningful housing, recognizing that housing is healthcare. Your work will extend beyond the office as you build relationships with landlords, identify housing opportunities, coordinate placements, and advocate for individuals throughout their journey to permanent housing.

Responsibilities

  • Assess housing readiness and teach essential independent living skills
  • Identify, secure, and support placement into permanent, recovery-oriented housing
  • Assist individuals through every step of the housing process—from applications to move-in
  • Conduct housing outreach, build landlord relationships, and expand housing opportunities
  • Support access to housing resources such as Housing Bridge subsidies, Section 8, and PRA 811
  • Help individuals maintain housing through coaching, advocacy, and ongoing in-home support
  • Mediate landlord-tenant concerns and promote successful tenancy
  • Assist with budgeting, household setup, lease compliance, and life skills development
  • Collaborate with community providers, housing agencies, and internal clinical teams
  • Maintain accurate documentation and electronic health records in compliance with Medicaid and regulatory standards

Requirements

  • Passion for working with individuals experiencing mental illness and housing instability
  • Strong communication, advocacy, and relationship-building skills
  • Ability to work independently in community-based settings
  • Organized, detail-oriented, and mission-driven
  • Experience with housing programs, case management, or outreach is a plus

Qualifications

  • Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, or Human Services required
  • One (1) year of human services experience preferred
  • Valid driver’s license, personal vehicle, and auto insurance required
  • Availability to work some flexible hours to provide on-call support and respond to after-hours concerns and emergencies is required
