Outreach Care Specialist (US)
Primary Duties
- Schedules appointments or enrolls members in programs based on follow-up care plan needs.
- Assesses member compliance with medical treatment plans through telephone or on-site visits.
- Identifies barriers to plan compliance and coordinates resolutions.
- Identifies opportunities impacting quality goals and recommends process improvements.
- Recommends treatment plan modifications and determines the need for additional services, collaborating with case management and providers.
- Captures referrals to local, state, or federally funded programs.
- Encourages members to reduce health risks and educates them on preventive measures.
- Prepares reports to document case and compliance updates.
- Maintains relationships with agencies contracted for services.
Requirements
- A high school diploma and at least 1 year of related experience; or a combination of education and experience that provides an equivalent background.
- Preferred qualifications include certification as a Certified Nurse Assistant or Certified Medical Assistant, a bachelor's degree in a related field, bilingualism, and mental health experience.
- Strong oral, written, and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills are essential.
