ROLE OVERVIEW and PURPOSE: The Outreach and Enrollment Counselor will conduct outreach and in-reach, educate, and assist clients enrolling in affordable health insurance coverage through Covered CA, Medi-Cal, Medicare, or the Children’s Health Insurance Program as well as non-health insurance programs such as Cal Fresh. Schedule Monday through Friday, 8 AM - 4:30 PM This position requires availability to work evenings and weekends as needed to support community events/meetings. While the standard schedule for this role is 40 hours per week, flexibility is provided and weekday hours can be adjusted when weekend/evening work is required. RESPONSIBILITIES: Conducts outreach activities and presentations at school and community events such as health fairs, churches, farmers markets, grocery stores, and local businesses; events may be scheduled during weekday, evening or on weekendsNetworks and establishes community relationships and partnershipsUnderstands and complies with monthly and yearly end goals and objectives in the various grant contracts and maintains complete data for monthly reportingSupports NHC efforts for patient retention as well as increasing new patient growthPresents health concepts to individuals and small groupsDisseminates information about affordable health insurance coverage to families and individualsConduct in-reach phone calls to current patients to offer enrollment assistanceProvides enrollment assistance in affordable health insurance coverage through Covered CA, Medi-Cal and the Children’s Health Insurance Program as appropriateProvides education and enrollment assistance for the CalFresh programProvides education on other community resources and assistance programs, such as SDG&E CARE programAssists the applicant with questions regarding health insurance, healthcare access and other resources and ensures enrollment process is completedRefers applicants to their medical home and other needed resources at time of application completionReceives referrals from collaborating and partner agenciesPerforms inter-departmental referrals for patient in need of medical, dental, and behavioral health services or other available servicesProvides coverage updates to clients regarding their affordable health insuranceAttends meetings and trainings necessary for the position sustainment and growthPrepare monthly quantitative and qualitative report on outreach/in-reach and application assistance activitiesShares feedback from patient experiences to improve access to health careKeeps self and others informed of organizational activities and promotes mission and goalsConducts patient referrals to assigned Medicare agents as requiredAssists in the efforts to retain patients and support efforts at eliminating patient attrition EDUCATION/EXPERIENCE: High school/GED requiredCovered California certification required within 90 days of employmentOne year of experience in government or community program eligibility/screening for Medi-Cal or Medi-Cal for families preferredCertified Enrollment Counselor for Covered CA experience preferredValid CA driver’s license and proof of auto insurance requiredBilingual (English/Spanish) highly preferred ADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities): Excellent verbal and written communication skills, including superior composition, typing, and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with health care and social service resources in the community preferredAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical manner Physical Requirements Ability to lift/carry 50 lbs/weightAbility to stand for long periods of time Pay Range: $27.21 - $38.09 per hour, depending on experience Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
